Community Youth Concepts resides at 1446 MLK Jr Pkwy, Des Moines, IA 50314, nestled within the heart of Des Moines. This central location provides a vibrant hub where youth from the community converge to engage in transformative programs aimed at nurturing lifelong skills and fostering personal growth.

Community Fridge & Pantry

At Community Youth Concepts, we are committed to supporting our community through our community fridge and pantry, located in the white shed on the west side of our building. Generously stocked with donations from caring individuals and organizations, the fridge and pantry offer essential supplies to those in need. Additionally, as our garden yields fresh produce, we proudly share this bounty by placing it in the white shed for community members to freely take what they require. This initiative embodies our dedication to fostering a caring and supportive environment for all.

To file a program discrimination complaint, a Complainant should complete Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: www.usda.gov/sites/default/files/documents/ad-3027.pdf, from any USDA office, or by calling 1-866-632-9992 or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to:

  1. USDA by mail: U.S. Department of Agriculture Office of the Assistant Secretary for Civil Rights, 1400 Independence Avenue SW, Washington, D.C., 20250-9410; or

  2. By fax: (833) 256-1665 or (202) 690-7442; or

  3. By email: program.intake@usda.gov.
